Open the public Web form below and enter your account email, platform, and optional details. After you submit the registration, we will process and delete the account within seven business days.
Accounts used on iOS, Android, macOS, and Windows can be registered for deletion through this page. We will process the deletion after the registration is submitted.
Open the public registration formKeep the edit link provided by Google Forms after submission. You can edit your own registration online, but editing does not trigger deletion.
Registering a deletion request does not cancel an Apple, Google Play, Creem, or other external subscription. Manage billing separately with the original provider.

Registration and deletion process
The form records the account email, selected platform, and optional details that you choose to submit. Responses are kept in a private sheet and are not shown to other respondents.
After you submit the registration, we will process and delete the account within seven business days. The form does not remotely erase local copies on offline devices.
Registering a request does not cancel Apple, Google Play, Creem, or another external subscription. Continue to manage billing with the original provider.
Registration result
The form displays a registration confirmation. We will process the account deletion within seven business days after submission.
Use the edit link provided by Google Forms after submission if you need to change your own registration; editing changes the registration information only.
